Teatterissa luovan uransa aloittanut Clive Barker (s.1952) ponnisti kerralla kauhumaailman huipulle vuosina 1984−1985 julkaistuilla Veren kirjat -antologioilla. Stephen King ylisti tulokasta kauhun tulevaisuudeksi. Kosmisen kauhun perinteestä ammentavat hittikirjat avasivat brittilupaukselle uusia ovia.

1970-luvulla Barker ohjasi lyhytelokuvia. Underworld on ensimmäinen pitkä elokuva, jonka käsikirjoittamiseen hän osallistui. George Pavloun ohjauksessa kirjailijalle ominainen alakulttuurikiinnostus yhdistyy David Cronenbergin tieteiskauhua muistuttaviin ideoihin.

Hämäräperäinen biokemisti (Denholm Elliott) kehittelee uutta hallusinogeeniä, joka aiheuttaa käyttäjilleen epämiellyttäviä mutaatioita. Kun varjoissa lymyävä hirviöjengi kaappaa suositun prostituoidun (Nicola Cowper), tämän entinen rakastaja (Larry Lamb) aloittaa matkan maanalaiseen Lontooseen.

Underworldin sinihehkuinen ja tyylikkäitä yksityiskohtia pursuava unimaailma muistuttaa 1980-luvun musiikkivideoiden estetiikkaa.  Coolin pinnan alla kuplii Barkerin uran läpäisevä visio maan alle tuomituista ihmisistä, joiden vaihtoehtoyhteisössä on enemmän asennetta ja kunniantuntoa kuin korruptoituneessa normiyhteiskunnassa.

Sivuroolissa elokuvan auktoriteettitasoa nostaa pahisikoni Steven Berkoff, joka oli juuri tehnyt uransa suurimmat elokuvat (Octopussy – mustekala, 1983, Beverly Hills kyttä, 1984, Rambo 2 – taistelija, 1985).  Barkerin ja Pavloun yhteistyö jatkui Rawhead Rex – Raakalaisessa (1986).
